RELEASE AND WAIVER FROM LIABILITY

Please read and sign this form, and return it to Vermont Bird Tours before attending an outing.

I understand that a nature outing presents enjoyment as well as obvious and necessary risks. These include, but are not limited to: insect bites or stings, snake bites, poison ivy, Lyme disease, seasickness, slipping and falling, a sprained ankle or broken bones, vehicle accidents, frostbite, or even a sore neck from looking at warblers high in the trees.

I am aware that an accident or illness on an outing could occur in a remote location without immediate access to medical treatment. Moreover, I am in good health and able to participate in Vermont Bird Tours outings. I understand that many risks are beyond the control of Vermont Bird Tours.

In signing this document, I voluntarily assume the risks of injury or property damage, and release Vermont Bird Tours, its owner, employees, agents, and contractors from any and all liability for personal injury or property damage resulting from negligence or any cause. I have read and understand the terms of this waiver, and agree that its terms apply to any trip I may take with Vermont Bird Tours in 2008.
